Who we are

Partnering for Health Services (PHS), an affiliate of Altoona Regional Health System, is a nonprofit medical clinic providing access to high quality health care to the uninsured residents of Blair County.

PHS provides patients access to primary care services, diagnostic services, medications, and referrals to specialists within its network. PHS is a cooperative effort between Altoona Regional, volunteer physicians and our patients.

Eligibility guidelines

• Patients must be residents of Blair County.
• Patients must NOT have insurance for the services they are applying for (this includes private insurance, Medical Access /Medicaid, Medicare, Veterans Benefits).
• Patients with access to employer sponsored health plans are NOT eligible to receive services.

Eligibility Income must be at or below the following levels, based on your household size (effective 2012):

*Symetra Insurance provides limited coverage for hospital services such as admissions and emergency department visits. It does not provide comprehensive medical coverage and is therefore not a substitute for major medical or any other comprehensive health care insurance coverage. The coverage provided by Symetra is limited to services obtained locally with participating providers such as Altoona Regional. Eligible patients will be offered the option to purchase Symetra at PHS. For patients whose income is greater than 200% of the Federal Poverty Guideline, Symetra will cover the out-of-pocket cost of ancillary services, diagnostic testing, most medications, and access to referrals within the PHS network. An intake appointment must be scheduled to become eligible for services. Please bring the following with you to your appointment:

• Photo ID (driver's license, passport, other government issued ID)
• W-2 Form for the most recent tax year.
• Copy of last year's tax return or Letter of Extension.
• Proof of family gross income for household members for the most recent month
• Medical Assistance denial letter
